Application

    If so, you may fill out an application.

The online application must be filled out on a computer, not a cell phone.

To fill out an application, you may either fill it out directly on this page in the form box below or fill out by hand. If you only have a cell phone we highly advise you fill the application on paper and send in via email, fax or mail. Click HERE for a copy of the application to print.

Please ask your regular veterinarian to send us your animals' medical records. If you do not have a regular veterinarian please supply any medical records you have of your animal's past.

Physical Application: print out form HERE. Then, fill out your application by hand. Send your completed application and supporting financial documents and a copy of your picture identification to our email address (themitzvahfund1818@gmail.com),

fax it to us (802-229-1911) or send it to us through the postal mail to our PO Box 56 East Montpelier, VT 05651.

Online Application: instead of printing, you may fill out an application in our form box below. Our online application is completely secure and streamlined for ease of use. Please note that the online application requires a desktop or laptop computer.

The Mitzvah Fund provides non-emergency preventative care for animals in need with qualifying owners. The veterinary staff assists good pet owners that are trying the best they can and cannot afford the necessary medical care their pet needs. We have a sliding fee program, and we encourage Mitzvah Fund recipients to pay it forward. That donation will help your animal or a neighbor's animal. What we do is all about supporting our entire village. ​

You will only be able to apply for assistance one time in a calendar year unless special circumstances have been reviewed and approved by the Mitzvah Fund fiscal review board.

Eligibility for the Mitzvah Fund's services will be determined through application review by our small staff.

Are you a low income veteran, senior (age 55+), or disabled first responder? Are you currently unhoused and working with a case manager or shelter?​ If so, you may qualify for our services.

​

In order to determine if you qualify, simply fill out our online form and follow the instructions carefully. We ask for sensitive information, and while our form is completely secure, be aware that using shared public computers could make your information visible to others. Whatever you are not comfortable filling out online, you can mail  to us or send by fax.

Please be as clear as possible in your answers and verify that there are no errors before submitting.
